Community Health Improment Process

The Community Health Improvement Process (CHIP) brings together many diverse partners to improve the health of our community. They align their efforts and support each other for greater collective impact on the priorities identified through our Community Health Assessment.

CHIP collaborators work together to identify priority issues, develop and implement strategies for action, and establish accountability to ensure measurable health improvement, which is outlined in the form of a Community Health Improvement Plan. CHIP looks outside of the performance of an individual organization serving a specific segment of a community to the way in which the activities of many organizations contribute to community health improvement. A new Community Health Assessment will be completed this year with a new report coming out in December 2015 that will review new data, emerging issues and define new or revised priorities for 2015-2018.

  • Healthy Living (Physical activity, healthy eating, and clinical weight management)
  • Tobacco (cessation and prevention)
  • Preconception Health
  • Early Childhood Development
  • Clinical-Community Connections

The Community Health Improvement Process is driven by two primary groups:

The Community Health Improvement Process Advisory

The Advisory provides leadership, support, and coordination to assist the community Workgroups established around each priority. Members are responsible for being advocates for systems, policy and environmental change in the community. Workgroups

Workgroups are composed of representatives from organizations that are currently working on the priority areas, community members, and interested Advisory embers. Health Improvement Specialists, through a contract with Mountain Area Health Education Center (MAHEC), provide support in convening the Workgroups. Representatives from the Workgroups report regularly to the Advisory Council to share actions, emerging issues and recommend policies for consideration.

The first product of their work together is a Community Health Improvement Plan. A CHIP Plan documents a long-term, systematic effort to address identified health problems in a community. This plan is used to set priorities and coordinate and target resources. A CHIP is critical for developing policies and defining actions to target efforts that promote health.

CHIP Contents:

  1. Goals, objectives, strategies & related performance measures
  2. Realistic timelines for achieving goals & objectives
  3. Designation of roles for HHS, hospital and other partners in CHIP implementation
  4. Required demonstration of organizations’ commitment to these roles
  5. Emphasis on evidence-based practices

The Community Health Improvement Plan for Buncombe County
