In Office Record Purchase

To purchase a death certificate in person, please bring your valid photo ID and payment to our office at 35 Woodfin St. Certified records can only be issued to immediate family or an authorized agent. Each certified record costs $10.00, and non-certified records cost $0.25 each.

Online Record Purchase

To purchase a record online, please go to the Online Vital Records Request page.

Request Record By Mail

To purchase a death certificate by mail, please send a copy of your valid photo ID, payment, and the completed Vital Records Request Form to:

Each certified record costs $10.00, and non-certified records cost $0.25 each. A stamped, self-addressed envelope can help ensure faster return.
