Social Work Division

We provide social work services including outreach, tracking, support, case management and mental health counseling to pregnant and postpartum women, at risk families with young children, clinic patients and Medicaid children for the purpose of infant mortality reduction, maximizing health and development and improving functionality and quality of life. We also provide Interpreter Services to non-English speaking patients receiving services at the Health Center.

 

 

Social Work Services include:

 

  • Child Service Coordination (CSC) - Provides support, counseling, and guidance to help ensure children ages 0 to 5 reach their developmental potential, are school-ready, and are raised in healthy, safe, nurturing homes.
  • General Social Work - Services for patients referred from Health Center clinics.
  • Health Check Coordination (HC) - Provide tracking and educational services to Medicaid eligible children (birth to 21 years) to update and reschedule health care needs. They promote well childcare and immunizations in the community.
  • Maternity Care Coordination (MCC) - Provides case management assistance for maternity and postpartum women with education, referrals to community resources, Medicaid and other applications, supportive counseling, and making and keeping their prenatal appointments.
  • Maternal Outreach Workers (MOW) - Provide in-home prenatal and parenting education and support services to high-risk Medicaid eligible pregnant women and mothers with infants to one year old.
  • Clinical Social Work Services - Mental Health services for adult Health Center patients and children referred by a Health Center or primary care physician in the community.
  • Bilingual Transportation Coordinator - Provides coordination of Health Center related transportation needs for Spanish-speaking patients.

How to get an appointment for Social Work Services


Social work services can be arranged by calling 250-5299. Any one can call to make a referral and a social worker will be assigned to contact the client. Mental health counseling services may be requested by calling 250-5299 but a physician referral will also need to be obtained. Interpreter Services are provided as a part of other Health Center appointments.

How are services paid for?

The Social Work Division accepts Medicaid insurance for case management services and there is no charge to clients who do not have Medicaid.

Clinical Social Work services may be paid through Medicaid, Medicare, North Carolina Health Choice, private insurance or on a sliding fee scale.
