  • Confidential 40 question health assessment
  • Personal results sent directly to employee
  • Group report gathered and presented to company
  • Follow-up and counseling on results available for employees

Comprehensive Wellness Program $55.00 per employee per year
  • Initial company consultation
  • Coordination of health screening
  • Confidential bi- annual HRA for each employee
  • Confidential follow up to individuals to discuss assessment
  • Group aggregate results to company
  • Distribution of educational materials
  • One class per month to suit the needs of your employees
  • Ending program evaluation
