• A W-2 Form containing the full 9 digit Social Security number
• A paycheck stub containing the full 9 digit Social Security number
• Military Discharge containing the full 9 digit Social Security number
• Medicaid/Medicare Card containing the full 9 digit Social Security number
• Voter's Registration Card containing the full 9 digit Social Security number
• Health Insurance Card containing the full 9 digit Social Security number
