Placement Services assists aging or disabled individuals who have no one available to assist them in finding substitute homes or residential health care facilities suitable to their needs when they are unable to remain in their current living situation.

Placement/Adult Foster Care Services include:

  • Initial screening/assessment
  • Assisting client/representative in securing needed medical information
  • Assisting with completion of necessary financial application
  • Help locate and secure placement in suitable setting and level of care

Hours of Operation:

Monday through Friday
8:00 a.m. - 5:00 p.m.

To make a referral call our Intake line at 250-5814 Monday through Friday 8:00 a.m. - 5:00 p.m..

How are services paid for?

No fee for this service.
