General Statement of Duties
The Public Health Nurse II provides professional nursing services to clients in disease control activities, preventive health and outreach (HIV counseling/testing), TB support services, STD and Immunizations. Client education, counseling, motivation, and utilization of community resources are major responsibilities. The PHN II conducts investigative interviews, counsels and teaches clients, responds to telephone calls from clients and community partners, makes appropriate referrals, provides needed immunizations, and applies public health nursing principles in working with the community to reduce the incidence and prevalence for infectious disease. This position also requires rotating On-Call responsibilities #(1941)
Must be licensed to practice as a Registered Nurse in North Carolina.
Successful completion of NC State Communicable Disease course is preferred.
Examples of Work
This position provides direct public health nursing services; services include assessment, education and non-judgmental counseling, administering medications and treatments, developing individualized plan of care, and coordination of community resources. Interview clients to update history, request laboratory tests, diagnostic procedures and immunizations as per Buncombe County Department of Health Standing Orders. Establish and maintain an accurate database through extensive health history of client; prioritizes areas of concern and communicates findings via client medical record or other appropriate document. Assess client’s physical, mental and social conditions. Interview clients and caregivers to assure that the client understand his/her diagnosis and the recommended treatment, and has resources so that he/she may adhere to the plan. Considerable client education and counsel is required. Make referrals to community agencies as well as community physicians, and prepares and instructs the client about the referral. Provide immunizations for vaccine preventable diseases and education of parents/clients regarding immunization laws, schedules, and care following immunizations. Assesses client’s needs related to immunization, provides education, counseling and referrals and administers appropriate immunization. Included in this are both required and recommended vaccines, and vaccines needed for travel abroad. Enter record of vaccination into NCIR and M&M. Provide follow-up on all reportable infectious diseases as appropriate. Provide clients and contacts with control measures necessary to prevent spread of disease as required by North Carolina General Statutes. Assures clients understand plan of care and the importance of compliance. Provide Public Health services at BCDH outreach events.
Knowledge, Skills and Abilities
Knowledge of public health and nursing services are essential. The ability to relate well with people including clients of all ages, caregivers of clients, professional and clerical staff, and outside agencies; skills in observing, listening, organizing, and prioritizing and communicating are required. Ability to efficiently and effectively interact with co-workers to accomplish common tasks; ability to have efficient and courteous interaction with the employer’s customers; ability to function in highly stressful circumstances; ability to maintain high level of professionalism and to conduct business in an ethical manner at all times.
Minimum Education and Experience
Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or Master’s in Public Health and graduation from a school of professional nursing and one year of professional nursing experience; or graduation from a school of professional nursing and two years of professional nursing experience including one year of Public Health Nursing experience; or an equivalent combination of training and experience.
